When completing the sacrament of Confirmation, children are to choose ONE sponsor who walks with them along their faith journey. They are to choose an adult (18 years or older) who is baptized in the Catholic faith and has received Holy Communion, and Confirmation. It cannot be the mother or father of the child. The sponsor must be present at the Confirmation Mass. Many children choose one of their Baptism godparents. Please complete the following information about your child’s sponsor. * Submitting a copy of your child's Baptismal Certificate is a requirement for participating in the confirmation program. Please send a copy of your child's Baptismal Certificate to the coordinator at confirmation@stmauriceparish.com. If you do not have a copy of the Baptismal Certificate, immediately contact the parish where your child was baptized to have a copy mailed to you. Please type "I understand."

* There is a $60.00 fee to cover the cost of resources and materials. Please complete the payment within one week of submitting this registration form. You can pay through the parish website at https://www.stmauriceparish.com/Confirmation. *Please add your child's full name in the Memo line.* Once the payment is complete, please forward your receipt to confirmation@stmauriceparish.com. Note: If this fee is prohibitive, contact the coordinator to make arrangements for a reduced fee. IMPORTANT: The registration is only complete once the PAYMENT RECEIPT and BAPTISMAL CERTIFICATE have been received by the coordinator by email. Please type "I understand."

* Photographs of My Child may be taken during the program, particularly at the Confirmation Mass. I authorize the Parish to use them for promoting its Sacramental programs.

* Each session is essential and integral to the program, and therefore I commit to attend ALL the sessions: Session 1: Sunday January 18, 2026 (2:00-4:00 pm) Session 2: Sunday February 8, 2026 (2:00-4:00 pm) Session 3: Sunday March 8, 2026 (2:00-4:00 pm) Session 4: Sunday April 19, 2026 (2:00-4:00 pm) Confirmation Mass & Rehearsal: Both dates TBA. * In the case of an absence due to a serious conflict, illness or emergency, the parent assumes the responsibility to contact the coordinator to discuss the next steps. This will include making-up for a missed session at home. An unjustified absence could result in being unable to complete the program and in delaying the reception of the Sacrament of Confirmation.
